Aunty Jean's Good Health Team: listening to the voices of the elders to create an Aboriginal chronic and complex care program : participatory evaluation of the Illawarra Health, Aboriginal Chronic and Complex Care, Pilot Program
Aunty Jean's Good Health Team began as a pilot program that aimed to develop a combined model of health promotion, education and self management that could be supportive of and sustain the development of good health behaviours and strategies for Aboriginal people with chronic and complex care needs.
